If you are a new client, the New Patient Intake Forms are required to be reviewed and completed prior to your initial visit or consultation. If you are unable to access these forms please contact our administrative staff.
New Patient Intake Forms
- Please include insurance card information if you intend to bill through insurance. If you do not have a physical card, please provide the number and group ID at the bottom of the profile sheet.
- If you have a general question abut insurance acceptance or availability prior to completing the intake, you can email Christina at Christina@viewpointpw.com
- Please be sure to check email and texts periodically as that will be a primary way of contactng you if we are unable to connect via phone.
